• Visit a variety of clients in their own homes, helping them recover from injuries, illnesses, or hospitalizations.
• Case manage, develop care plans, and deliver skilled care, including assessments, medication management, and wound care.
• Progress clients from discharge to meeting their goals at home, providing education to patients and their families as needed.Requirements:
  • Active and current RN license
  • One year of clinical experience preferred
  • Previous home health care experience welcomed, but not required. We'll train you.
